Dustin Schloemp, CEO is a licensed occupational therapist and co-founder of Ascension Rehab Care, a mobile outpatient therapy practice dedicated to expanding access, convenience, and quality of care for patients in their homes and communities. With extensive experience across outpatient, home-based, and post-acute care settings, Dustin brings a patient-centered and outcomes-driven approach to rehabilitation.
Driven by a vision to empower clinicians, Dustin helped establish Ascension Rehab Care to create a model where therapists have greater ownership of their work, professional autonomy, and long-term sustainability. He believes that when therapists are supported, accountable, and valued, patients receive better care and experience better outcomes.
Dustin is passionate about innovative care delivery models that remove traditional barriers to therapy, improve continuity of care, and foster strong partnerships with physicians, senior living communities, and healthcare organizations. His leadership focuses on clinical excellence, ethical practice, and building a culture that prioritizes growth, collaboration, and accessibility.

Adam Kendig, COO is a licensed physical therapy assistant and co-founder of Ascension Rehab Care, a mobile outpatient therapy practice focused on delivering high-quality rehabilitation services directly to patients in their homes and communities. With a strong clinical background and experience across multiple care settings, Adam is committed to improving patient outcomes through individualized, functional, and evidence-based care.
Adam co-founded Ascension Rehab Care with a shared vision of empowering therapists while expanding access to therapy services for patients who face barriers to traditional outpatient care. He believes that bringing therapy into the home allows for more meaningful treatment, improved compliance, and better long-term results.
As a leader, Adam is dedicated to fostering a culture of accountability, professional growth, and sustainability.
