About Us

Ascension Rehab Care was founded by two experienced rehabilitation therapists who identified a growing need to improve access to high-quality outpatient therapy—particularly for patients who face barriers to attending traditional clinic-based care. Transportation challenges, mobility limitations, caregiver constraints, and scheduling difficulties often prevent patients from receiving the therapy they need. Ascension Rehab Care was created to eliminate these obstacles by delivering care directly to patients in their homes and community-based settings.

Our founders envisioned a mobile outpatient practice designed by therapists, for therapists, where clinicians are empowered to take ownership of their work while providing convenient, patient-centered care. By bringing therapy to the home, senior living communities, and other residential settings, our therapists are able to deliver personalized, functional interventions that are immediately relevant to patients’ daily lives—improving carryover, safety, and independence.

Ascension Rehab Care provides therapists with a platform that promotes autonomy, accountability, and sustainability while supporting flexible scheduling and responsible caseload management. This model allows clinicians to practice at the top of their license without the volume-driven pressures of traditional clinics, enabling them to spend meaningful time with patients and focus on outcomes rather than throughput.
